Key Clinical Responsibilities:
- Clinical Monitoring: Administer daily medications; conduct regular symptom checks (tracking for headaches, vomiting, or vision changes) and provide OTC medication as needed in accordance with instructions from physicians.
- Proactive Symptom Assessment: Every 1-2 hours, evaluate the patient's comfort and administer OTC medications as needed.
- Safety & Fall Prevention: Provide assistance when walking as needed, especially when navigating stairs and thresholds.
- Hygiene & Personal Care: Supervise showers/grooming; provide regular "prompted voiding" reminders for restroom use.
- Cognitive Support: Use redirection for distractibility and personality changes.
- Documentation & Family Communication: Maintain documentation of medications administered, cognitive condition and daily activities, communicate updates to family members
Key Household & Aide Responsibilities:
- Meal Management: Plan and prepare all daily meals (Breakfast, Lunch, Dinner).
- Errands & Logistics: Manage grocery shopping and household errands.
- Transportation: Drive the patient to and from medical appointments (vehicle provided or mileage reimbursed).
- Light Household Duties: Kitchen cleanup after meals; patient-specific laundry (linens/clothes).
Requirements:
- Current Oregon LPN License.
- Experience in Neurology, Oncology, or Memory Care preferred.
