Job Description:
<div>The primary focus of this position will be cardiac surgery nurse navigator.<br><br>&nbsp;Manages comprehensive nursing care for patients (within specialty area) whose needs range from uncomplicated to complex and rapidly changing. Success Criteria:<br>&nbsp;Based on holistic assessment, identifies relevant aspects of the situation and determines the appropriate course of action to move patient/family towards optimal health.&nbsp;<br>&nbsp;Assesses changes in patient status; anticipates problems before they occur and implements measures to minimize risk.<br>&nbsp;Organizes and prioritizes and documents nursing care activities considering the needs of the patients and other staff members.<br>&nbsp;Provides leadership to develop, analyze, integrate complex data, and update individualized care.<br>&nbsp;Negotiates short/long term goals with the patient to develop and evaluate the individualized care.<br>&nbsp;Initiates/facilitates/collaborates with all members of the interdisciplinary team to improve care delivered to the patients, ensuring that appropriate referrals (interdisciplinary and community) are made and completed.<br>&nbsp;Utilizes the nursing process to facilitate the patient’s care using time and resources efficiently, and assumes accountability for the effectiveness of care. Coordinates care with the interdisciplinary members consistent with roles and responsibilities.<br>&nbsp;Initiates comprehensive patient and family education.<br>&nbsp;Uses professional judgement to appropriately delegate<br>&nbsp;Makes and documents, age-appropriate interventions based on individual patient needs.<br>&nbsp;Coordinates and manages care by applying analytical reasoning, reflection, rational problem-solving skills, using evidence-based information and clinical judgment.&nbsp;<br>&nbsp;Safely performs technical skills in area of practice. Demonstrates practice based on knowledge of all polices, procedures, nursing standards, and legal issues in areas of specialty.<br>&nbsp;Demonstrates a caring, compassionate approach to patient/family, functioning as a patient/family advocate<br>&nbsp;Participates in shared decision making and problem solving to promote effective collaboration with interdisciplinary team<br>&nbsp;Use established outcome measures to evaluate effectiveness of care.<br>&nbsp;Promotes effective and efficient use of educational resources in achieving desired patient outcomes.<br>&nbsp;Evaluates and documents patient’s response to treatment.<br>&nbsp;Collaborates with support services and multidisciplinary team in providing care.<br>&nbsp;Assures quality care through professional growth and development.<br>&nbsp;Serves in preceptor role for clinical area.<br>&nbsp;Uses formal and informal methods to teach other staff members..<br>&nbsp;Independently evaluates own practice and goals; initiates discussion of performance with manager.<br>&nbsp;Provides written professional practice goals for the upcoming year.&nbsp;<br>&nbsp;Monitors progress toward, and provides evidence of achieving mutually agreed upon goals within established time frame. Provides written evaluation of previous year’s goals.<br>&nbsp;Attends to own continuing education by providing evidence of a minimum of six (6) contact hours; or two relevant academic courses per year; or certification in specialty area with evidence of contact hours required to maintain certification.&nbsp;<br>&nbsp;Demonstrates effective use of computer-based information for decision support, continuous quality improvement and professional development<br>&nbsp;Assumes leadership role in clinic area and the larger organization.<br>&nbsp;Takes action and coaches others in resolving ongoing problems and unit issues.<br>&nbsp;Develops and implements recommendations to improve the unit environment.<br>&nbsp;Provides effective representation on a committee, task force, or unit council.<br>&nbsp;Evaluates, and develops recommendations for new and/or revised standards of nursing care and practice in response to patient or unit needs.<br>&nbsp;Effectively manages conflict by confronting the issue or problem and collaborating with others to explore and implement acceptable solutions.<br>&nbsp;Demonstrates effective performance in an identified practice focus by providing written or verbal evaluation of projects related to practice focus.<br>&nbsp;Demonstrates fiscal accountability for one’s own practice while providing quality care.<br>&nbsp;Participates in Performance Improvement/Research activities designed to improve patient care, and/or organizational processes.<br>&nbsp;Reviews and analyzes current nursing literature and research in area of specialty as a basis for improving practice.<br>&nbsp;Identifies clinical problems for study, communicates these to appropriate resources and seeks opportunities to participate in investigative/evaluative activities.<br>&nbsp;Participates in Performance Improvement activities/clinical research and provides written summary of involvement.<br>&nbsp;Collects data for evaluation to improve care.&nbsp;</div>

Minimum Qualifications:
<div>Graduation from state-accredited school of registered nursing and two years of nursing experience, with at least one year in the assigned specialty area. Must have a current license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing. All degrees must be received from appropriately accredited institutions.</div>
