Maximus is seeking a compassionate and detail-oriented Clinical Assessment Coordinator to join our team. In this role, you will review clinical assessments to ensure completeness and accuracy, summarize key findings, and recommend appropriate rehabilitative services and placements based on individual needs. Your strong understanding of mental health diagnoses, symptoms, and medications will be essential in providing high-quality, person-centered recommendations that support positive outcomes for each individual.
About the Program:
The Indiana Level of Care Assessment and Referral (IN LCAR) program is a comprehensive statewide initiative encompassing several key components, including the PASRR (Preadmission Screening and Resident Review). This program mandates that all individuals being considered for admission to a Medicaid-certified nursing facility, irrespective of their payor source, undergo a comprehensive screening for Serious Mental Illness, Intellectual Disabilities, or Related Conditions. The objective is to prevent inappropriate institutionalization by ensuring that each individual's specific needs are accurately identified and addressed.
*This remote position will support Central Standard Time (CST) work hours.

Essential Duties and Responsibilities:
- Responsible for completing medical and/or behavioral health assessments within contract requirements
- Conducts on-site, video call and/or telephonic assessments to determine an individual’s needs for services and supports, eligibility, level of care or related outcome
- Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
- Travel may be required based on program contract requirements
- Performs other related duties as assigned
Required Skills/Abilities:
- Preferred knowledge may include community support programs, long-term care assessment and level of care in medical, behavioral health or related programs
- Knowledge and understanding of medical and/or behavioral health diagnoses and prescribed medications
- Ability to collect data, define problems, establish facts, and draw valid conclusions
- Apply clinical judgment and decision-making skills to evaluate information and make accurate, timely determinations
- Review clinical documentation within a highly regulated healthcare environment while adhering to strict turnaround-time requirements
- Identify missing, conflicting, or incomplete documentation and obtain appropriate clarification to support accurate assessments and decisions
- Analyze complex information, apply critical thinking, and resolve issues while maintaining a high level of accuracy and attention to detail
- Balance quality, compliance, and operational requirements to ensure timely and appropriate outcomes
Minimum Requirements:
- Education and licensure requirements are based on program contract requirements and are outlined in job posting
- High School Degree or equivalent required
- Minimum 1 year of clinical experience required
- Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field OR a master’s degree in any discipline OR an active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license
- Minimum of two (2) years of professional experience in social work, human services, behavioral health, care coordination, or a related field OR two (2) years of nursing experience
- Strong multitasking skills with the ability to prioritize and manage a high volume of tasks
- Proficiency in Microsoft Office Suite (Word, Outlook, Excel) and comfort working with various digital tools
- Ability to work independently and as part of a team in a fast-paced environment
- Strong written and verbal communication skills, including the ability to interact professionally with internal teams and external providers
Preferred Requirements:
- Experience reviewing medical records, History & Physicals (H&Ps), hospital records, behavioral health documentation, and care plans
- Ability to identify indicators of Serious Mental Illness (SMI), Intellectual Disability (ID), Developmental Disability (DD), and Related Conditions (RC)
- Understanding of nursing facility placement criteria and community-based service alternatives
- Knowledge of PASRR (Preadmission Screening and Resident Review) requirements and federal nursing facility regulations
- Working knowledge of Medicaid-funded Long-Term Services and Supports (LTSS) and nursing facility admission processes
