Same exceptional care, same dedicated teams—fully connected.Step into a career and join a team whose mission is to move lives forward every day!!About the RoleWe are seeking a compassionate, dedicated, and proactive Direct Support Professional (DSP) to join our Community and Outpatient Services program . In this role, you will play a vital part in implementing rehabilitation strategies and providing guidance, support, and encouragement to individuals working toward personal independence in their home and community settings. May include supporting clients at doctors appointments or other activities to promote health, wellness and stabilityFrom assisting with daily living activities to supporting vocational and recreational pursuits, you will empower clients to build confidence, achieve their personalized goals, and thrive in a safe, home-like environment.Essential Duties & ResponsibilitiesClient Support & Rehabilitation- Promote Independence: Assist, guide, and encourage clients with Activities of Daily Living (ADLs), community living skills, and personalized rehabilitation goals.Health & Wellness: Assist with medication administration as assigned, monitor nutrition plans, and prepare well-balanced meals daily.Proactive Behavioral Support: Support clients through challenging situations (e.g., refusal of activities or medications) and implement behavior management plans while maintaining professional boundaries and mutual trust.Observation & Advocacy: Identify changes in clients' behavioral, emotional, cognitive, or physical conditions and alert the treatment team to ensure timely care.Transportation & Community Outings: Safely transport and accompany clients to medical appointments, group outings, errands, and recreational activities.
- Compliance & Record Keeping: Accurately complete required shift documentation—including daily data sheets, goal sheets, vital/seizure flow sheets, supervision checklists, and incident reports—in compliance with CARF, state, and federal standards.Environment & Safety: Maintain a clean, organized, safe, and welcoming living environment for residents in alignment with licensing requirements.Team Communication: Participate in shift hand-offs, review operational communications (emails, meeting minutes, rounds summaries), and collaborate effectively with family members, care teams, and leadership.
Qualifications & Requirements
- Education: High school diploma or equivalent required. Operational Requirements: Valid driver's license with a record of responsible driving. Must have the ability to travel and/or operate a vehicle for up to 20% of the time spent working in this position.Physical Demands: Ability to lift up to 50lbs.
